Possible to add count-logica / skip first X pics of album

I'm trying to figure out the following: I'm building a "case" page like a portfolio. I want it to open with a big picture, then have one/two pictures on the left side and some text on the right side, then continue with all the other pictures from the album.

And I can't figure out if it's even possible in the koken mark-up. I'd need something like this:
- koken:first to get the first image
- two column layout
- koken:load 2nd/3rd pic
- Load text content
- koken load 4th + pic

I've looked into the limit-variable which is nice, but it doesn't enable me to say, load a album loop starting from the forth picture onwards.
I've also looked in if/else logic markup if I could do something like "if count > 4" to skip the first three in a loop, but I can't find the "count" logica markup in the documentation.

In PHP this is very simple stuff, so I imagine koken can do it aswell, but can anybody tell me how?
