How do I specify an image to use through editing html?

I'm trying to create a custom bio page and want to include an uploaded image at a certain size. The information you have in your how-to docs talk about using something like <koken:img width="250" height="250" crop="true"></koken:img> however there's never a mention of where the image actually resides on my system. I see the uploaded image refered to in [koken_upload filename="my-image.jpg" label="my-image.jpg"] but this is outside the koken:img tag and so I cannot specify the file size.

I don't want to create a whole new theme for just one bio page. It would be nice to have some documentation showing how to use css and the koken tags to control just a single page.
